Children's Targeted Case Management

Children’s Targeted Case Management (TCM)

Children’s Targeted Case Management assists families and children with behavioral health diagnosis, intellectual disability, and/or a serious medical condition. Our Case Managers support families as they assess and prioritize the needs of the family and child. Case Managers focus on the strengths, needs, and individual goals developed as part of an individual treatment plan.

Case Management

Advocates for clients’ needs with service providers, schools and other organizations
Develops an individualized service plan with clients, guardians and families
Monitors the effectiveness of services, natural supports and resources
Promotes independence in order to maintain the least restrictive environment for children to be active members of their families
Identifies and coordinates referrals and resources to target any needs that the youth and/or family may have
Builds relationships and forms partnerships to enhance care
Manages behavioral health issues, intellectual disabilities and severe medical conditions

Under 21 years of age; if over 18, enrolled in public school
Documentation of either a mental health diagnosis, developmental disability or a chronic medical condition from a licensed professional
Diagnosis must be considered eligible under the most recent edition of the Diagnostic and Statistical Manual of Mental Disorders, or a diagnosis described in the current version of the Diagnostic Classification of Mental Health and Developmental Disabilities of Infancy and Early Childhood

We provide services in Kennebec, Somerset, Franklin, Waldo, Lincoln, Knox, Sagadahoc and Penobscot Counties!

Children's Behavioral Health Home

The Behavioral Health Home (BHH) is a service that offers a team based approach for a child and family to comprehensively address physical health and mental health issues. This service is delivered by a team of qualified professionals that include: medical and psychiatric consultation, health home coordinators, a clinical team leader, nursing, peer/family supports and frequent collaboration with the member’s primary care provider. The BHH provides a primary emphasis on preventative health and wellness, in addition to community partnering.

Assists With The Following:

Develops individual treatment plans with the member and their parents and/or guardians
Participates in ensuring the delivery of crisis intervention and resolution
Offering alternatives to hospitalization
Coordinates services with primary care provider, schools, and any other organizations
Assures continuity of care by assessing gaps in services by coordinating referrals and resources
Organizes team meetings to review progress

Eligibility Criteria:

Newborn through age 20
Documentation of a mental health diagnosis that is under one year old and is considered eligible under the most recent edition of the Diagnostic and Statistical Manual of Mental Disorders, or a diagnosis described in the current version of the Diagnostic Classification of Mental Health and Developmental Disabilities of Infancy and Early Childhood
