Case Management
Children’s case management services improve the quality of lives of children and families with medical, social, educational, and psychological barriers. An individualized care plan is created with emphasis on developing resources to improve outcomes in education, respite, medical, rehabilitative, counseling, and social settings.

Case management services consist of family-focused treatment planning, coordination, and monitoring of services, crisis planning, prevention, emotional support, advocacy, resource linkage, and community education.

Assistance Plus provides case management services to children from 0 to 21 years of age. The child must be Mainecare eligible and have a mental retardation or mental health diagnosis. Parents, service providers, teachers, or other professionals directly involved with the child’s care may make a referral for service.

Services are presently provided in Androscoggin, Franklin, Kennebec, Knox, Lincoln, Oxford, Sagadahoc, Somerset, and Waldo counties.
