Targeted Case Management for Children (Section 13)

Children’s Targeted Case Management (TCM) assists a family whose child has behavioral health issues, intellectual disabilities, and severe medical conditions. Our children’s case manager supports the family in assessing and prioritizing the family’s needs. The case manager develops a unique treatment plan for each family and child that identifies the child’s and family’s strengths, needs, and individualized goals.

The case manager locates resources in the client’s community such as family therapy and outpatient behavioral health services, and monitors the effectiveness of these programs. The case manager advocates for the child’s needs with schools service providers, and medical professionals. The overall goal of the case manager is to help the family use their voice to advocate for their child.

Our case management team is trained in crisis intervention and CPR/First Aid. In addition to these trainings, case managers are provided with quality clinical supervision and training opportunities intended to build their knowledge of the latest practices in working with youth.

Eligibility Criteria

  • Active Mainecare or Katie Beckett coverage
  • Under 21 years of age
  • active mental health diagnosis or intellectual disability diagnosis
  • Major medical condition impacting the child’s functional ability
